The goal of this project is to describe the design, implementation and evaluation of innovative partnerships among health, social care and community sectors to advance the wellbeing of older adults at a population level.
In our first project, we describe an innovative program led by Jenna Davis, MHM, BA, R/TRO, in which North Simcoe Muskoka Specialized Geriatric Services led an initiative in which adult day programs in the region partnered with Barrie’s Bradford Greenhouse to develop and implement a seasonal gardening workshop series with the goal of increasing access to recreational programs for older adults living with dementia and other complexity.
Our study describes the process involved in establishing the partnership, the intervention (gardening workshop), and evaluation findings. We highlight broader impacts in terms of population health and health system efficiencies.
